Abstract

Background: Postoperative urinary tract infection (UTI) remains a significant complication following ureterorenoscopy for stone management, which contributes to patient morbidity. Despite the use of antibiotic prophylaxis in routine, infection rates still persist, warranting investigation into adjunctive intraoperative strategies.

Objective: To determine the efficacy of intraoperative antibiotic irrigation in preventing post ureterorenoscopy urinary tract infection in patients with ureteral stones.

Methodology: One hundred patients undergoing ureterorenoscopy for ureteral stones were included. Out of 100 patients, seventy five received irrigation with normal saline containing gentamicin (80mg in 3L), while 25 received normal saline irrigation only. Efficacy was assessed after 15 days of the procedure, defined no sign of clinically and microbiologically confirmed UTI. Data was analysed with SPSS 23.

Results: Mean age of the patients was 39.13±11.43 years. The mean operative time was 58.06±8.18 minutes and mean stone size was 8.41±1.33 mm. Efficacy was achieved in 64 (85.3%) patients in the antibiotic irrigation cohort, while 8 (32.0%) patients achieved efficacy in the normal saline cohort. This difference was statistically significant (p < 0.001). Common isolated pathogens were Pseudomonas aeruginosa, Escherichia coli, and Klebsiella species.

Conclusion: Intraoperative antibiotic irrigation during URS, reduces the risk of postoperative UTI in patients with ureteral stones. Antibiotic irrigation can be effective in reducing UTI.
